Dr. Phillip A. Jakubowicz, DDS
Dr. Phillip Jakubowicz (“Dr. J.”) is from Middlebury, Indiana and graduated from Northridge High School. He then attended Trine University, where he double majored and graduated Magna Cum Laude. After completing his undergraduate degree, he received his Doctorate from Indiana University School of Dentistry in 2013.
His previous work history includes serving as a Navy dentist for 6 years with tours in South Carolina, Okinawa, Japan, and San Diego, California. He continues to serve as the 180th Fighter Wing's dentist in the Ohio Air National Guard.
He has completed additional training in implant placement, 3rd molar extractions (wisdom teeth), and clear aligner therapy, and he continually pursues educational opportunities to better serve his patients.
He loves spending time with his wife Brandi and his two children, Stella and Bode. He is an avid DIYer and woodworker and also enjoys exercising and playing board games. Dr. Jakubowicz works out of the Middlebury Dental Office and looks forward to serving the community he is proud to call home.
